In 1998, three years after defeating the T-1000 and averting the rise of the malevolent artificial intelligence (AI) Skynet, Sarah Connor and her teenage son John are relaxing on a beach. A T-800 Terminator, sent from the future by Skynet, arrives and shoots John, killing him.

In 2020, a cybernetically-enhanced young woman named Grace, and a more advanced Terminator model, the Rev-9, are separately transported from the future to Mexico City. The Rev-9 infiltrates a factory where Daniella "Dani" Ramos works alongside her brother Diego. It attempts to kill Dani, but is thwarted by Grace, who escapes with Dani and Diego in a car. The Rev-9 pursues them in a truck, and reveals its ability to separate into two separate entities: a powerful endoskeleton, and a liquid-like metallic exterior. The Rev-9 ends up killing Diego and cornering Grace and Dani, but then Sarah arrives and temporarily disables both parts of the Rev-9 with explosives. Afterward, Grace begins to collapse. She reveals that her augmentations grant superhuman abilities but only for short bursts, and she is reliant on various medicines to survive. After acquiring medical supplies, Dani, Grace, and Sarah retreat to a motel.

Grace reveals that she has been sent from 2042 to protect Dani. Sarah believes that, like her, Dani is destined to give birth to the future Resistance leader. Sarah says that she knew where to find them because in the years since John's death she has received messages detailing the arrivals of Terminators, which she then destroys; each message ends saying "For John". Grace notes that neither Skynet nor John exist in her future. Humanity is instead threatened by an AI called Legion, designed for cyberwarfare. In this altered future, Legion seizes control of technologies, disabling entire cities, and humanity unsuccessfully tries to neutralize it by detonating nuclear devices to create an electromagnetic pulse (EMP). Legion then builds a global network of machines to exterminate the human survivors.

Grace traces the messages to a location across the Mexico-United States border. The Rev-9 tracks the group by connecting to the CCTV surveillance network, and surmises they will cross the border. It infiltrates the US border patrol and arranges for the group to be arrested and brought to its location. The Rev-9 attempts to kill Dani, but she, Grace and Sarah escape on a helicopter. Traveling to the source of the messages, they discover the T-800 that murdered John. After fulfilling its objective, it was aimless and adopted a family, integrating into, and learning from, humanity, eventually developing a conscience, and coming to be named Carl. It explains that it sent Sarah the messages to give her purpose. Requiring Carl's help, she promises to kill him once the Rev-9 is gone.

Rev-9 reacquires the group, forcing them to steal an aircraft from a nearby military base to escape. Aboard, Grace reveals that Dani is the future leader of the Resistance and Grace's adoptive mother; she will save Grace as a child and raise her after the apocalypse. The Rev-9 intercepts their craft in one of its own, destroying them both. Grace, Sarah, and Dani parachute to safety in a Humvee, while Carl and Rev-9 crash into a river beside a hydro-power plant. Rev-9 damages Carl and continues its pursuit of Dani. Grace stalls Rev-9, and Dani and Sarah knock it into the river's strong current. The group reunites with Carl and breaks into the plant, as Grace's power begins to fade. The group takes a stand against Rev-9, allowing Carl and Grace to force it into a spinning turbine that subsequently explodes. Carl, Grace, and Sarah are incapacitated.

Rev-9 survives but is severely damaged, having lost its liquid exterior entirely. Dying, Grace orders Dani to take her power source and use it as an EMP against Rev-9. Dani tackles Rev-9 but is subdued until Carl reactivates and restrains it. Dani stabs Rev-9 with Grace's power source, and Carl drags it and himself over a ledge into a pit, where the resulting electrical discharge melts both androids, destroying them.

 

Later, Dani watches Grace as a child; she tells Sarah that she is determined to avert Grace's death. Sarah tells her it is time to train as the pair drives off together.
